By signing this form, the Student irrevocably accepts all study conditions at Azerbaijan Medical University and educational institutional partners and all Ellevitta Limited contractual conditions.
Legal mention: I do agree by signing this document electronically and submitting and/or sending it by email, fax, mail or any other mean, that all the information submitted are correct and I accept the terms and conditions of studies including all contractual conditions of this enrollment form.
General terms and conditions for successful applicants:
1-1. The Applicant irrevocably accepts all the conditions outlined in the Acknowledgement Letter and agrees to join the TRC Value-Added Program, including networked medical studies at Azerbaijan Medical University (AMU) and the additional BS/MPH package provided by the Representative (if offered a seat). The Applicant understands that he cannot opt out of any part of the package or request any price reduction if he does not wish to follow or does not pass any of the Ellevitta Value added PROGRAM.
2- The Representative is to provide the Student a study seat in AMU if the Student fulfills all requirements imposed by the administration of AMU and this form.
3-The Applicant accepts and should not contest the fact that his offer is personal. All financial details mentioned in the “Agreement” may differ for different applicants for reasons the Representative deems appropriate, such as additional entry intake, academic year, other citizenship, etc. The Representative has complete discretion regarding it.
4-The Representative will guide the Student to complete his/her admission.
5-The Representative is not to be held responsible for any false document provided by the Student.
6—If the Student opts for an included accommodation program, he/she is offered Public/Private hostel-type accommodation in either two—or three-seater rooms, depending on availability, upon his/her arrival to AMU.
7- The Applicant undertakes that if he/she prematurely terminates his studies at AMU and/or this Contract, the Student shall not apply for a position of Study at AMU without the prior written approval of the Representative.
8- If the Applicant breaches the undertaking in Clause 7 above, the Applicant shall be liable to pay damages to the Representative equal to 2 years’ full fees relating to the Ellevitta Value added PROGRAM.
9- Clause 8 shall survive the termination of the contractual agreement. It shall apply for two years from the date of termination of the Student’s initial period of Study or the Date of termination of this Contractual agreement (whichever is later).
10- The Accommodation charges are covered according to the academic year and not on a monthly basis.
11- The Student is to transfer the annual Fee package payments only to Ellevitta Limited or its representative bank account for including indemnification of training, including accommodation charges before the beginning of each academic year and failure to do so he/she will not get a permission slip to attend classes, and this may lead to the expulsion from the AMU program.
12- Suppose the University directs a student to repeat any academic semester or an academic year. In that case, the Student must pay the relevant charges to Ellevitta Limited or its representative bank account as in any other academic year before the repeating session starts.
13- The Student is to abide at his/her full responsibility with Azerbaijan laws, AMU, and his agreement with Ellevitta Limited.
14- The Student admits that if he/she is expelled from Azerbaijan Medical University due to academic failure, misbehavior, or any other reason, he/she cannot request a refund.
15- All financial matters, such as payments and refunds, should be discussed with Ellevitta Limited through the Representative email.
16- The only payment made directly to the University’s bank account is if the Student caused any damage, penalties, or extra classes that he/she requests.
17- The Applicant is aware that provided information by the applicants, including personal details filled in the Enrolment Form or any other documents or emails or web forms, are for a need-to-know basis and to access and facilitate the applicant admission to Azerbaijan Medical University and can be used for this purpose by subcontractors or agents of Azerbaijan Medical University or/and Ellevitta limited and its subcontractors.
18- Any conflict regarding these terms and conditions should be solved in a friendly manner.
24- These terms are valid for the whole period of the student studies at AMU/
25- If requested by Ellevitta Limited upon admission, the Student must sign the three parties’ Contract with the Representative and the University. Under this exclusive agreement, all parties, including the applicant, agree that Ellevitta Limited must receive full fees before the start of each year.
THIS ENROLLMENT contains Stand-alone and/or ADDITIONAL CONTRACTUAL OBLIGATIONS TO THE AGREEMENT SIGNED BY THE APPLICANT, AND Ellevitta limited Forms must be signed by the Applicant personally. Unsigned forms will not be processed.

Applicant/prospective students should mail all required documents* filled and signed with complete pages to Ellevitta Limited. Failure to do so application process will be discontinued.
